- Tobacco Last Cited Date: Most recent date tobacco use was noted.
- Tobacco Last Used Date: Last known date of tobacco use by the applicant.
- Tobacco Type: Type of tobacco products used.
- Tobacco Use Frequency: Details of tobacco usage over the last five years, broken down annually.
- Health and Medical History
- Family History: Notable health conditions in the applicant's family.
- Significant Impairments: Any major impairments affecting the applicant.
- System-Specific Conditions: Detailed records for various bodily systems, such as,
Cardiovascular, Endocrine, Dermatology, Gastrointestinal, Genitourinary, Hematology, Immunology, Musculoskeletal, Neurology, Oncology, Ophthalmology, Psychiatric, Respiratory, Nephrology.
- Lifestyle and Habits
- Habits: Lifestyle choices or habits that may impact health.
- ADL (Activities of Daily Living): Ability to perform daily activities independently.
- ER Visit: Records of emergency room visits.
- Occupation: The current or past occupation of the applicant.
- Avocation: Hobbies or activities that might affect health.

- Page Count: Number of pages in the report.

How is BMI calculated in the PIA?
We've improved the BMI formula to enhance data representation:
• If there are BMI entries from the last year, we calculate the average of those.
• If there’s nothing from the last year, we will display the most recent BMI value in the case.
• If there are no dates with BMI data, we will calculate an average of all available data.
